Q-Orchestrate adds `/orchestrate <request>` — a dispatcher that decomposes a
request into a SQLite ledger of tasks and runs scoped sub-agents (scout,
devils-advocate, backend-dev, frontend-dev, devops, cybersecurity,
qa-engineer) against them in git worktrees. `/orchestrate-status` prints the
current ledger state via `qo status`.

**Never run `/orchestrate` automatically.** It is `disable-model-invocation:
true` for a reason — only invoke it when the user explicitly asks to run it.

The ledger (runs, tasks, events) is the source of truth, not any markdown
file. Do not hand-edit ledger state; use the `qo` CLI.

`qo` is already on PATH (plugin bin/); run every `qo` command from the
project root as a single plain command — never `cd`, never chain with `;`,
`&&` or `|`, never look for the plugin directory.
